After-Care Guide

Welcome home! ๐ŸŽ‰ Your transplant is a fresh start. Following these steps carefully will help keep your new kidney healthy and you feeling your best. Please read this guide with a family member so you always have support.

 

๐Ÿ’Š  1.   Your Medications

 

These medications protect your kidney. Never skip or stop them without talking to your doctor first.

๐Ÿ’ก Set phone alarms for 8:00 AM, 8:30 AM, and 8:00 PM so you never miss a dose!

 

  • Monitor your blood pressure every day before taking your BP medicines
  • Target Tacrolimus blood level: 7โ€“10 ng/mL (your doctor will guide you)

 

๐Ÿฅ—  2.   What to Eat & Drink

 

โœ…  Foods that are safe for you:

 

  • Well-cooked chicken, fish, and boiled eggs (up to 4โ€“5 eggs/day)
  • Milk is fine
  • Boiled or purified water only

 

โŒ  Foods to avoid:

 

  • Raw or uncooked foods, outside fruits, salads, and fresh juices
  • Junk food, smoked food, and processed items
  • Red meat and curd/yogurt
  • Grapefruit, pomegranate, and pomelo โ€“ these interfere with your medication!

 

๐Ÿšถ  3.   Staying Active

 

  • Take a short walk every day โ€“ start slowly and build up
  • Use your breathing exercise device (incentive spirometry) for 6 months
  • Wear your DVT compression stockings during the day
  • Do NOT lift anything heavier than 5 kg for the first 6โ€“8 weeks
  • Stay away from crowded places; if you must go, wear two masks
  • Do NOT ride on two-wheelers until your DJ stent is removed (Day 21)

 

๐Ÿงผ  4.   Personal Hygiene

 

  • Wash your hands often with soap and use sanitizer regularly
  • Brush your teeth twice a day and keep your mouth clean
  • Use a separate toilet if possible
  • Change your clothes every day
  • Change bed sheets every 1โ€“3 days
  • Use your own utensils and towels โ€“ don't share
  • Stay away from pets, dusty areas, and anyone who is sick

 

๐Ÿ“Š  5.   Tracking Your Health at Home

 

  • Keep a daily intake-output chart (how much you drink vs. how much urine you pass) for 6 months
  • Weigh yourself every morning at the same time
  • Call your doctor if your weight changes by ยฑ5 kg from your usual 'dry weight'

 

๐Ÿ’ก Keep a simple notebook or use a free phone app to track your weight and fluids each day.

 

๐Ÿ“…  6.   Your Follow-Up Schedule

 

Please never miss your follow-up visits. Your doctor catches problems early this way!

๐Ÿšจ  7.   Call Your Doctor Immediately If You Notice:

 

  • Swelling in your face, hands, or legs
  • Fever, nausea, or vomiting
  • Much less urine than usual
  • Difficulty breathing or chest tightness
  • Pain, redness, or discharge at your surgical site

๐Ÿ’‰  8.   Vaccinations

 

  • Always check with your doctor before getting any vaccine
  • You must NOT receive live vaccines such as BCG, MMR, or Typhoid vaccine

 

โ˜€๏ธ  9.   Sun Protection

 

  • Apply SPF 50+ sunscreen before going outdoors
  • Wear long sleeves, a hat, or carry an umbrella when outside
๐Ÿ’ก Your immunosuppressive medications can make your skin more sensitive to the sun. This is normal โ€“ just protect yourself!

 

โš ๏ธ  10.   Medication Safety Rules

 

  • If you forget a dose, take it as soon as you remember โ€“ but NEVER double up on the next dose
  • Avoid painkillers like Ibuprofen or Naproxen (NSAIDs) โ€“ they can harm your kidney
  • Do NOT take any herbal or traditional medicines without asking your doctor first

 

๐Ÿ’™  11.   Your Emotional Well-Being

 

Recovery is a journey, and it's completely normal to have ups and downs. Be kind to yourself.

 

  • The steroid medication (Prednisolone) can sometimes cause mood changes or irritability โ€“ this is a known side effect
  • Talk to your family, friends, or a counselor if you feel low or anxious
  • Your care team is here to help you โ€“ don't hesitate to reach out

 

โค๏ธ  12.   Sexual & Reproductive Health

 

  • You can resume sexual activity after 6โ€“8 weeks
  • Always use protection (condoms) and maintain good hygiene
  • Women: Avoid pregnancy for at least 1.5 years after transplant
  • When you are ready to plan a family, speak with your doctor first โ€“ some medications may need to be adjusted before conception

 

๐Ÿงช  13.   Getting Your Blood Tests Done

 

  • Go to the lab between 7:45 AM and 8:00 AM
  • Do NOT take your Tacrolimus dose before the blood sample is taken
